Generic Name:

Indications / Used For:
 Asthma
 Maintenance treatment of Chronic Obstructive Pulmonary Disease (COPD)
Mechanism of Action:
 Budesonide:
A glucocorticosteroid with dose-dependent anti-inflammatory action in the airways.
Inhaled use leads to reduced symptoms and fewer exacerbations, with fewer side effects
than systemic corticosteroids. The exact anti-inflammatory mechanism is not fully
understood.
 Formoterol:
A selective β2-adrenoceptor agonist producing rapid (within 1–3 minutes) and longlasting (≥12 hours) bronchodilation in patients with reversible airway obstruction.
Dosage Form:
 Inhalation via Rotacaps containing Formoterol Fumarate and Budesonide.
Side Effects:
 Common:
Oropharyngeal Candida infections, headache, tremor, palpitations, throat irritation,
coughing, hoarseness.
 Uncommon:
Aggression, anxiety, psychomotor hyperactivity, sleep disorders, dizziness, tachycardia,
nausea, bruising, muscle cramps.
 Rare:
Hypersensitivity reactions (e.g., urticaria, dermatitis, angioedema, anaphylaxis),
hypokalemia, cardiac arrhythmias, bronchospasm.
 Very Rare:
Cushing's syndrome, adrenal suppression, growth retardation, bone mineral loss,
hyperglycemia, depression, behavioral changes, cataract, glaucoma, QTc prolongation,
angina, BP fluctuations.
Drug Interactions:
Avoid concurrent use with:
 CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, HIV protease inhibitors)
 Beta-blockers (including eye drops)
 Antiarrhythmics (e.g., quinidine, disopyramide)
 Tricyclic antidepressants, MAO inhibitors
 L-Dopa, L-thyroxine, oxytocin, alcohol
 Non-potassium-sparing diuretics
Contraindications (When NOT to Use):
 Known hypersensitivity to Budesonide, Formoterol, or any excipient
 As primary treatment for status asthmaticus or acute asthma/COPD episodes
Precautions:
 Monitor for glaucoma, cataracts, or vision changes, especially in long-term use.
 Patients with history of increased intraocular pressure or ocular disorders should be
regularly evaluated.
Warnings:
1. Not for acute asthma/COPD attacks. Not a rescue inhaler.
2. Do not use with other LABAs (e.g., salmeterol), risk of cardiovascular complications
and fatalities.
3. Systemic steroid effects like adrenal suppression or hypercorticism may occur in high
or sensitive users—taper off slowly if needed.
