Metformin HCl (Biguanide):

🔄 Drug Interactions
•    Diuretics → Risk of volume depletion

•    Cimetidine, ranolazine, dolutegravir, vandetanib → May affect metformin levels
•    Zonisamide, acetazolamide, corticosteroids → Risk of lactic acidosis

🎯 Indication
•    For adult patients with Type 2 Diabetes Mellitus who:
o    Are inadequately controlled with either empagliflozin or metformin alone
o    Are already treated with empagliflozin + metformin separately
•    Used to improve glycemic control
•    Not indicated for Type 1 Diabetes

🚫 Contraindications (When Not to Use)
•    Renal impairment or end-stage renal disease (ESRD), including patients on dialysis
•    Metabolic acidosis, including diabetic ketoacidosis
•    Hypersensitivity to empagliflozin or metformin

🛑 Warnings
1.    Lactic Acidosis (Metformin-related):
o    Rare, but serious and potentially fatal
o    Early symptoms: Malaise, myalgia, abdominal pain, respiratory distress
o    Risk factors: Renal impairment, dehydration, alcohol abuse
2.    Hypotension (Empagliflozin-related):
o    Due to intravascular volume contraction
o    Common in elderly, renally impaired, or diuretic users
o    Monitor blood pressure and hydration status
3.    Ketoacidosis (Empagliflozin-related):
o    Potentially life-threatening
o    Symptoms: Nausea, vomiting, abdominal pain, labored breathing
o    Discontinue if suspected; not for use in type 1 diabetes
