•    Heart failure (NYHA class II-IV)
•    Post-myocardial infarction care (to improve survival)

How It Works:
Valsartan is an angiotensin II receptor blocker (ARB).
It works by:
•    Blocking angiotensin II from binding to the AT1 receptor, which causes:
o    Reduced vasoconstriction
o    Decreased aldosterone secretion
o    Lower blood pressure
•    Acts independently of the pathways for angiotensin II synthesis.

Dosage:
•    Valsartan (strength and regimen as prescribed by a physician)

Side Effects:
Common:
•    Viral infections
•    Dizziness, vertigo, postural hypotension
•    Neutropenia
•    Hyperkalemia
•    Renal impairment
Uncommon:
•    URTI, pharyngitis, sinusitis
•    Insomnia, decreased libido
•    Syncope, hypotension
•    Diarrhea, abdominal pain
•    Fatigue, asthenia, back pain, edema
Rare:
•    Dizziness (isolated)

Drug Interactions:
Avoid or use with caution with:
•    Lithium
•    NSAIDs (including COX-2 inhibitors and high-dose aspirin)
•    Transporter inhibitors (e.g. rifampin, ciclosporin)
•    Efflux transporter inhibitors (e.g. ritonavir)
•    Aliskiren (in diabetic or renally impaired patients)

Indications:
•    Hypertension
•    Heart failure (especially when ACE inhibitors are not tolerated)
•    Post-myocardial infarction with evidence of left ventricular dysfunction

When Not to Use:
Do not use valsartan in:
•    Known hypersensitivity to valsartan or excipients
•    Severe hepatic impairment, biliary cirrhosis, or cholestasis
•    Pregnancy (2nd and 3rd trimesters)
•    Use with aliskiren in patients with diabetes or renal impairment (GFR < 60 mL/min/1.73 m²)

Precautions:
•    Primary hyperaldosteronism: not recommended
•    Use cautiously in hepatic impairment without cholestasis

Warnings:
1.    Potassium Monitoring:
Avoid potassium-sparing diuretics, potassium supplements, or potassium-rich salt substitutes. Monitor potassium levels.
2.    Renal Artery Stenosis:
Caution in bilateral renal artery stenosis or solitary kidney – monitor renal function.
3.    Liver Impairment:
Use caution in patients with mild to moderate hepatic dysfunction.
