How It Works
Metformin HCl:
An antihyperglycemic agent that:
Decreases hepatic glucose production
Decreases intestinal glucose absorption
Improves insulin sensitivity by enhancing peripheral glucose uptake and utilization
Empagliflozin:
An SGLT2 inhibitor that:
Reduces renal glucose reabsorption
Lowers the renal threshold for glucose
Increases urinary glucose excretion

Empagliflozin (≥5% incidence):
Urinary tract infections

Indication
For adults with Type 2 Diabetes Mellitus:
Not adequately controlled with either empagliflozin or metformin alone
Already being treated with both agents

When Not to Use
Do not use in cases of:
Renal Impairment, ESRD, or patients on dialysis
Metabolic acidosis, including diabetic ketoacidosis
History of serious hypersensitivity to empagliflozin or metformin

Store in a cool, dry place, away from direct heat and light

Warnings
Lactic Acidosis
Rare but potentially fatal
Nonspecific symptoms: malaise, myalgia, abdominal pain, drowsiness
Can lead to hypothermia, hypotension, bradyarrhythmias
Hypotension
Due to volume depletion from empagliflozin
Monitor in patients with renal impairment, elderly, or those on diuretics
Assess and correct fluid status before initiating
Ketoacidosis
Serious condition, requires hospitalization
Can occur even in type 2 diabetics
Not for use in type 1 diabetes mellitus
