How It Works
Empagliflozin is an SGLT2 inhibitor:
SGLT2 is a protein in the kidneys responsible for reabsorbing glucose from urine back into the blood
By inhibiting SGLT2, empagliflozin:
Reduces renal glucose reabsorption
Lowers the renal threshold for glucose
Increases urinary glucose excretion
Lowers blood glucose levels

Side Effects
Very Common:
Hypoglycemia (especially when used with sulfonylureas or insulin)
Common:
Vaginal yeast infections (moniliasis), vulvovaginitis
Balanitis and other genital infections
Urinary tract infections
Generalized itching (pruritus)
Increased urination
Uncommon:
Volume depletion
Dysuria (painful urination)
Increase in blood creatinine or decrease in glomerular filtration rate
Rare:
Diabetic ketoacidosis (DKA)

Drug Interactions
Diuretics, especially thiazide and loop diuretics
Other SGLT2 inhibitors
Concomitant use may increase the risk of dehydration, hypotension, and electrolyte imbalances

Indication
Monotherapy: For patients intolerant to metformin
Add-on therapy: With other antidiabetic agents, including insulin, when glycemic control is not achieved through those alone
Used in adults with type 2 diabetes mellitus, alongside diet and exercise

When Not to Use
Known hypersensitivity to empagliflozin or any of its excipients
Severe renal impairment, end-stage renal disease (ESRD), or on dialysis

Precautions
Intravascular volume contraction may cause symptomatic hypotension, especially in:
Elderly patients
Those with renal impairment
Patients on diuretics
Patients with low systolic blood pressure
Monitor fluid status and correct volume depletion before initiating therapy

Warnings
1. Risk of Hypoglycemia
Increased when used with insulin or sulfonylureas
Consider dose reduction of insulin or insulin secretagogues
2. Risk of Urinary Tract Infections (UTIs)
Including urosepsis and pyelonephritis requiring hospitalization
Monitor patients and consider discontinuing in recurrent UTI cases
3. Diabetic Ketoacidosis (DKA)
Not indicated for type 1 diabetes or DKA treatment
Discontinue immediately if DKA is suspected
Interrupt therapy during major surgery or acute serious illnesses, and restart when stabilized
