How It Works
Fenofibrate is a fibric acid derivative that acts primarily by activating the Peroxisome
Proliferator Activated Receptor alpha (PPARα). Activation of this receptor:
 Enhances lipolysis and removal of triglyceride-rich lipoproteins
 Increases the activity of lipoprotein lipase
 Reduces production of apoprotein C-III
 Increases synthesis of apoproteins A-I and A-II
These effects help lower VLDL and LDL (bad cholesterol), raise HDL (good cholesterol), and
reduce small dense LDL particles, improving the lipid profile in patients at risk of cardiovascular
disease.

When Not to Use
Do not use Fenoget in patients:
 With hypersensitivity to fenofibrate or its components
 With severe renal dysfunction
 With liver disease, including primary biliary cirrhosis
 With unexplained abnormal liver tests
 With pre-existing gall bladder disease
 With chronic or acute pancreatitis (except in cases related to severe
hypertriglyceridemia)
 During breastfeeding

Precaution
 Ensure lipid levels are persistently abnormal before starting treatment
 Always attempt lifestyle changes (diet, exercise, weight control) and treat contributing
conditions (e.g. diabetes, hypothyroidism) before medication
Warnings
Warning 1:
Regularly monitor liver function tests (LFTs). Discontinue if ALT (SGPT) remains elevated to
more than 3 times the normal range.
Warning 2:
Increased risk of gallstones due to higher cholesterol excretion in bile. If symptoms suggest
cholelithiasis, evaluate the gallbladder and consider discontinuing therapy.
Warning 3:
Discontinue treatment if muscle pain, tenderness, or weakness occurs and myopathy or
rhabdomyolysis is suspected. Check serum creatine kinase (CK) levels promptly.
