Used For:
 Chronic management of hyperuricemia in patients with gout
How it Works:
Febuxostat is a non-purine selective xanthine oxidase (XO) inhibitor. It lowers serum uric
acid levels by inhibiting XO, the enzyme responsible for converting:
 Hypoxanthine → Xanthine → Uric Acid
This reduction in uric acid helps prevent gout attacks.
Dosage:
 Febuxostat (dose individualized by physician)
Side Effects:
Common:
 Headache, diarrhea, nausea, rash, liver function test (LFT) abnormalities
Uncommon:
 Increase in blood amylase, creatinine, urea, LDH, triglycerides
 Decreased platelet count, hemoglobin
 Dizziness, somnolence, paraesthesia, altered taste
 GI symptoms (abdominal pain, reflux, vomiting, constipation, flatulence, dyspepsia)
 Nephrolithiasis, hematuria, frequent urination
 Skin reactions (dermatitis, urticaria, pruritus)
 Joint and muscle pain (arthralgia, arthritis, myalgia, cramps)
 Weight gain, increased appetite
 Hypertension, flushing, edema
 Flu-like symptoms, reduced libido
Rare:
 Palpitations, renal insufficiency, weakness (asthenia), thirst, nervousness, insomnia
Drug Interactions:
 Naproxen, NSAIDs, Probenecid
 Inhibitors of glucuronidation
 UGT enzyme substrates
 Contraindicated with azathioprine, mercaptopurine, theophylline
Contraindications (When Not to Use):
 Hypersensitivity to febuxostat or any excipient
 Patients using:

▪ Theophylline
 Asymptomatic hyperuricemia
Precautions:
 Not recommended in patients with ischemic heart disease or congestive heart failure
Warnings:
1. Gout Flares:
 Increased flares may occur after starting febuxostat due to urate mobilization
 Prophylactic NSAID or colchicine is recommended at initiation
2. High Urate Formation Conditions:
 Not recommended in conditions like malignancy or Lesch-Nyhan syndrome (risk of
xanthine urinary deposition)
3. Liver Monitoring:
 Liver function tests advised at 2 and 4 months, and periodically afterward
