Mechanism of Action
Esomeprazole is a proton pump inhibitor (PPI) that acts by irreversibly binding to the H⁺/K⁺
ATPase enzyme in gastric parietal cells.
 This enzyme system is the final step in acid production.
 Inhibition results in a profound and long-lasting reduction in gastric acid secretion,
leading to increased gastric pH and symptom relief.
Dosage
 As prescribed by the physician
 Usual adult dose: 20–40 mg once daily, depending on indication and patient response
Side Effects
Common:
 Headache
 Gastrointestinal disturbances: Nausea, abdominal discomfort, diarrhea, constipation
 Skin reactions
 Dry mouth
Rare:
 Angioedema
 Anaphylaxis
 Fundic gland polyps (especially with long-term use)
Drug Interactions
Esomeprazole may interact with:
 Antiepileptics: Phenytoin
 Anticoagulants: Warfarin

Contraindications
 Known hypersensitivity to esomeprazole or other proton pump inhibitors
 Severe allergic reactions such as:
o Angioedema
o Anaphylaxis
o Bronchospasm
o Acute interstitial nephritis
o Urticaria
Precautions
 Vitamin B12 Deficiency Risk: Long-term daily use (>3 years) may impair
cyanocobalamin (B12) absorption.
 Monitor B12 levels in patients at risk.
Warnings
1. Hypomagnesemia: Rare but serious cases reported with prolonged use. Consider
periodic monitoring.
2. Osteoporosis-related Fractures: Long-term/high-dose PPI therapy may increase risk of
fractures (hip, wrist, spine), especially in elderly or those with other risk factors.
3. Clopidogrel Interaction: Avoid co-administration due to possible reduction in
clopidogrel’s antiplatelet effect.
