How It Works:
Omeprazole is a proton pump inhibitor (PPI) that irreversibly blocks the H⁺/K⁺ ATPase
enzyme system (proton pump) in the gastric parietal cells. This action reduces both basal and
stimulated gastric acid secretion, regardless of the cause of stimulation.
Dosage:
Omeprazole (as per physician’s advice and condition severity)
Side Effects:
Common:
 Headache
 Diarrhea, constipation
 Abdominal pain, flatulence
 Nausea or vomiting
Uncommon:
 Dizziness, paraesthesia
 Insomnia, somnolence, vertigo
 Skin rash, pruritus, urticaria
 Increased liver enzymes
 Malaise
Rare:
 Mental confusion, agitation, hallucinations (mainly in severely ill)
 Aggression or depression

Drug Interactions:

 Immunosuppressants: Tacrolimus, Methotrexate
 Antibiotics: Clarithromycin
 Others: Phenytoin, Rifampicin, St. John's Wort, Erlotinib
Indications:
Omeprazole is indicated for:
 Treatment and prevention of erosive reflux esophagitis
 Long-term management to prevent GERD relapse
 Symptomatic relief in patients with non-erosive GERD
When Not to Use:
 Hypersensitivity to omeprazole or other benzimidazole derivatives
 Concurrent use with nelfinavir
Precautions:
 Long-term therapy may reduce vitamin B12 absorption; monitor in at-risk patients
 Consider periodic evaluation of magnesium and calcium levels in prolonged use
 May mask symptoms of gastric malignancy
Warnings:
1. Rule out malignancy in gastric ulcers before starting treatment — symptom relief may delay
diagnosis.
2. Long-term, high-dose PPI use (especially >1 year) may slightly increase risk of osteoporotic
fractures (hip, wrist, spine), especially in the elderly or those with other risk factors.
3. In patients with liver impairment, a dose reduction may be necessary due to increased
bioavailability and half-life.
