 Treatment of Gastroesophageal Reflux Disease (GERD)
🔹 How It Works:
Omeprazole inhibits gastric acid secretion by irreversibly blocking the hydrogen/potassium
ATPase enzyme (proton pump) in gastric parietal cells. This reduces both basal and stimulated
acid secretion regardless of the stimulus.

Omeprazole (as prescribed by a healthcare professional)
🔹 Side Effects:
✅ Common:
 Headache
 Diarrhea, constipation, abdominal pain, nausea/vomiting, flatulence
✅ Uncommon:
 Dizziness, paraesthesia, somnolence, insomnia, vertigo
 Increased liver enzymes
 Rash, pruritis, urticaria
 Malaise
✅ Rare:
 Reversible mental confusion, agitation, aggression, depression, hallucinations (mostly in
severely ill patients)
 Gynecomastia
🔹 Drug Interactions:
May interact with:
 Ketoconazole, itraconazole
 Diazepam, citalopram, imipramine, clomipramine, phenytoin
 Atazanavir, Digoxin, Clopidogrel
 Posaconazole, erlotinib, Saquinavir, Tacrolimus
 Methotrexate, clarithromycin
 CYP2C19 and CYP3A4 substrates (e.g., rifampicin, St John's wort)

🔹 Indications:
Omeprazole is indicated for:
 Treatment of erosive reflux esophagitis
 Long-term management of healed esophagitis to prevent relapse
 Symptomatic treatment of GERD
🔹 When Not to Use (Contraindications):
 Known hypersensitivity to omeprazole or substituted benzimidazoles
 Concomitant use with nelfinavir
🔹 Precautions:
 May reduce vitamin B12 absorption due to reduced stomach acid, especially with longterm use or in patients with low B12 stores
🔹 Warnings:
 Suspected gastric ulcers should be evaluated to rule out malignancy, as symptom relief
may delay diagnosis
 Long-term or high-dose use (>1 year) may increase risk of fractures (hip, wrist, spine),
especially in elderly or at-risk patients
 Dose adjustment may be needed in patients with impaired liver function due to increased
bioavailability and half-life
