How It Works
Omeprazole inhibits gastric acid secretion by irreversibly blocking the hydrogen/potassium
ATPase enzyme system (proton pump) in the gastric parietal cells. This effect reduces both basal
and stimulated acid secretion, regardless of the stimulus.

 Common:
o Headache
o Diarrhea, constipation, abdominal pain, nausea/vomiting, flatulence
 Uncommon:
o Dizziness, paraesthesia, somnolence, insomnia, vertigo

o Malaise
 Rare:
o Reversible mental confusion, agitation, aggression, depression, hallucinations

Indications
 Treatment of erosive reflux esophagitis
 Long-term management of healed esophagitis to prevent relapse
 Symptomatic treatment of GERD
When Not to Use (Contraindications)
 Known hypersensitivity to omeprazole or substituted benzimidazoles
 Concurrent use with nelfinavir
Precautions
 Omeprazole may reduce vitamin B12 absorption due to reduced stomach acid
(hypochlorhydria or achlorhydria). This is important for patients with low B12 stores or
those on long-term therapy.
Warnings
1. When gastric ulcer is suspected, malignancy should be ruled out as symptom relief may
delay diagnosis.
2. Long-term or high-dose use (over one year) may increase the risk of fractures (hip, wrist,
spine), especially in elderly or at-risk patients.
3. Dose adjustment may be necessary in patients with impaired liver function due to
increased drug bioavailability and half-life.
