How It Works:
Septran is a combination of Sulfamethoxazole and Trimethoprim that works synergistically to
inhibit folate synthesis in bacteria:
 Sulfamethoxazole blocks the conversion of para-aminobenzoic acid to dihydrofolic acid.
 Trimethoprim inhibits the enzyme dihydrofolate reductase, blocking the next step in
folate metabolism.
This dual-action kills or inhibits the growth of susceptible bacteria.
Usage and Safety
Dosage:
As prescribed by the physician depending on age, weight, and severity of infection. (Each 5ml
contains Sulfamethoxazole 200mg + Trimethoprim 40mg)
Side Effects:
 Gastrointestinal: Nausea, vomiting, diarrhea, abdominal pain, glossitis, dry mouth
 Hematologic: Leukopenia, thrombocytopenia, megaloblastic anemia, hemolytic anemia
(especially in G6PD-deficient patients)
 Musculoskeletal: Myalgia, arthralgia, rhabdomyolysis
 Dermatologic: Alopecia, rash
 Other: Fatigue, insomnia, epistaxis, edema, vision problems
Drug Interactions:
 Major Interactions: Warfarin, Methotrexate, Phenytoin, Zidovudine, Cyclosporin
 Others: Rifampicin, Thiazides, Pyrimethamine, Digoxin, Lamivudine, Azathioprine,
Repaglinide, Folinic acid
Indication:
Used to treat or prevent infections caused by susceptible bacteria. It is important that the use
is based on clinical evidence or strong suspicion of bacterial infection.
When Not to Use:
 Known allergy to Trimethoprim or Sulfonamides
 Megaloblastic anemia due to folate deficiency
 Pregnant women at term
 Nursing mothers
 Infants under 2 months of age
Precautions:
 Avoid use without confirmed or suspected bacterial infection to prevent antibiotic
resistance
 Monitor renal and liver function during therapy
 Monitor folate levels in high-risk groups (elderly, alcoholics, malnourished, those on
long-term anticonvulsants)
Warnings:
Warning 1
Use with caution in patients with renal or hepatic impairment, folate deficiency, or asthma.
Warning 2
Ensure adequate fluid intake to prevent crystalluria and kidney stone formation.
Warning 3
Antibiotic-associated diarrhea can occur during or after treatment. Contact a physician if
persistent or severe diarrhea occurs, especially if it contains blood.
Pregnancy Category:
