How It Works:
Tiotropium is a long-acting anticholinergic (antimuscarinic) agent. It blocks M3 muscarinic
receptors in airway smooth muscle, causing prolonged bronchodilation lasting over 24 hours.
Dosage:
 Adults: One 18 µg inhalation capsule once daily via dry powder inhaler (DPI) or
approved inhalation device
 Not recommended for children
Side Effects:
 Common: Dry mouth, headache, dizziness, constipation, sinusitis, pharyngitis
 Uncommon/Rare: Blurred vision, urinary retention, atrial fibrillation, tachycardia,
palpitations, paradoxical bronchospasm, oropharyngeal candidiasis, epistaxis, dysphonia,
taste disturbances, rash, insomnia, joint swelling, dry skin, dehydration, dental caries,
glossitis, gingivitis, stomatitis
Drug Interactions:
 Safe to use with β₂-agonists, methylxanthines, and inhaled or systemic corticosteroids
 Avoid combining with other anticholinergics due to increased risk of side effects such as
dry mouth, urinary retention, or blurred vision
When Not to Use:
 Known hypersensitivity to tiotropium or related anticholinergic compounds (e.g.,
atropine, ipratropium, oxitropium)
 Not intended for rescue therapy of acute bronchospasm
Precautions:
 Use with caution in patients with narrow-angle glaucoma, urinary retention, or prostatic
hyperplasia due to anticholinergic effects
 Monitor patients with renal impairment for side effects
Warnings:
1. Discontinue immediately if hypersensitivity reactions such as rash, angioedema, or
bronchospasm occur
2. Paradoxical bronchospasm is a rare but serious side effect; treat with rescue inhaler and
discontinue Spiridow
3. Avoid inhalation exposure of the powder to the eyes as it may cause irritation; ensure
correct inhaler technique
